The Coaching Services offered are not a substitute for professional mental health care or medical health care and are not intended to diagnose, treat, or cure any mental or medical disease or condition. Although the Coach is a licensed medical professional Certified Family Nurse Practitioner, they are not YOUR Primary Care Practitioner, you are responsible for communicating with your local, private Primary Care Provider. You are fully responsible for your health and well-being at all times while working with the Coach, including, without limitation, your choices and decisions. The Coach is in no way liable for your health or safety; If the Coach suspects the existence of any disease, disorder, or condition, then you will be informed of such (although you acknowledge that this is not a formal medical diagnosis). In which event, it is your sole responsibility to promptly seek medical care under a licensed physician or other healthcare professional. The Coaching Services offered are solely for the purpose of aiding you in achieving your health and wellness goals and do not constitute formal medical advice and outcomes and goal achievements are largely dependent on individual characteristics and are not guaranteed.

Waiver
You are voluntarily and fully assuming all risks, regardless of severity, that you may sustain in connection or associated with the health and wellness coaching provided to you by the Coach, including, without limitation, the specific Coaching Services described above. You, for myself and your heirs, assigns, personal representatives, and next of kin, expressly waives and releases any and all claims, now known or hereafter known, against the Coach and its successors, assigns, employees, contractors, officers, directors, agents, affiliates, subsidiaries, and parent companies, each and all of them (collectively, the "Releasees"), on account of personal injury, property damage, or otherwise arising out of or relating to the coaching services, including without limitation, acts or omissions of yourself or the Coach as a result of the advice given by the Coach or otherwise resulting from the coaching relationship. You will not to make or bring any such claim against any Releasee, and forever release and discharge all Releasees from any and all liability under such claims. All matters arising out of or relating to this waiver and release will be governed by and construed in accordance with the laws of the State of the Coach’s address as listed above (Pennsylvania), without giving effect to any choice or conflict of law provision or rule.  Any claim or cause of action arising under this waiver and release may be brought only in the federal and state courts located in the city and State of the Coach’s address as listed above, and you consent to the exclusive jurisdiction of such courts. You understand that this waiver and release is intended to be as broad and inclusive as permitted by law and that if any portion hereof is held invalid, you agree that the remainder will continue in full legal force and effect. You further agree that if this waiver and release is not valid as such in the State, then it will be construed as a covenant not to sue.  You further agree to defend, indemnify and hold harmless the Releasees from any and all claims arising or resulting from injuries or illness (including death), damages, or loss, including, but not limited to attorneys’ fees, sustained by yourself arising out of, connecting with, or in any way associated with your voluntary participation in the coaching services.
